Tattoo Booking Request Form

Please fill out the following form to the best of your ability. If you have more than one tattoo idea, please use a separate form.

Deposits are non-refundable/non-transferrable, I require 72 hours notice to change or cancel your appointment or you will lose the deposit. I may also require a new deposit if I have to make major changes to the design, so please be sure you have given me all the final details before booking.

Please note I don’t send out drawings ahead of time, but I do book extra time at the beginning of the appointment to go over the design and make any edits if needed (however I rarely have to make major changes if I have all the info I need :))

Please advise me of any allergies, medical conditions or concerns ahead of time so that I can best accommodate you. Masks are optional at this point, I'm happy to wear one if you prefer so just let me know what you are comfortable with. I also only have room for one person at a time so unfortunately can't accommodate guests in my space, only the person getting tattooed.
